Twin Oaks Convalescent Center

Long Term Care at its Finest

Twin Oaks Convalescent Center became a part of Bacon County Hospital and Health System in December of 1990 and was attached to the hospital in 1992. It is an 88 bed skilled nursing facility fully licensed by the State of Georgia, and is rated a Four Star Facility on Nursing Home Compare. It consistently receives high acclaim from our public and is regarded as one of the best facilities in South Georgia. Twin Oaks has an excellent activities program, and a very active Family Council and Resident Council. Their Resident and Family Councils, an organized group of residents' family members, are very involved with the resident care issues through interactions with staff, resident activities, and fund-raising efforts.

The residents participate in several annual including the Ms. Twin Oaks Beauty Pageant, National Nursing Home Week, 4-H sponsored Ms. Young at Heart, a Halloween Carnival and community parades just to name a few. All activities are extremely popular with Bacon Countians and well supported by local civic clubs.

Bacon County Hospital broke ground for phase one of their building/expansion program for the nursing home in January of 2013 and was completed in May of 2014. Phase one of the project included 20 private rooms and baths for the residents, a beautiful Chapel for the residents to worship and a new entrance facing Highway 32 East. The second phase will include six private rooms for a total of 28 private rooms. This phase will also include the Rehabilitation Department being expanded for residents receiving short and long term rehabilitation services, and will include a special hospice room for end of life issues that will accommodate the resident as well as their family.
