What is My BCH Portal?

My BCH Portal offers patients access to their personal medical record that is securely stored on the Bacon County Hospital network, and offers flexibility to access your health information and other Bacon County Hospital-related resources 24 hours a day, seven days a week.

Is there a fee to use My BCH Portal?

No. My BCH Portal is free and accessible to view your Bacon County Hospital medical record.

What are the enrollment options?

  • Hospital enrollment - hospital staff will create the account and provide you with a onetime userid and password

  • Self enrollment - a patient can self enroll using the self enrollment link as long as an email address has already been provided to hospital staff


  • How do I enroll?

    Register for an account using the self enrollment option. You must have already provided an email address to hospital staff and you must know your medical record number in order to chose this option. If you do not have all of the required items for registration, please visit the Medical Records Department. Sign in for the first time using the onetime userid and password provided to you by hospital staff.

    What are the self enrollment criteria?

  • The user must already be a patient and have a medical record number in the system.

  • Proxy users cannot use self enrollment (note: the HIPAA Privacy Act indicates that written consent is required for a Proxy user to access a patient's medical information)

  • The patient must have already provided an email address to hospital staff. The email address used to self enroll must match the email address given to hospital staff.

  • If you were already enrolled by hospital staff, the self enrollment option will not work. Please sign in here and enter the onetime userid and password provided by hospital staff.


  • Why do you need my email address?

    My BCH Portal requires an email address as a part of patient enrollment. This email address is included in your patient profile and is used to confirm your enrollment login credentials.

    Is My BCH Portal secure?

    My BCH Portal ensures your health information is kept secure. My BCH Portal uses industry best practices for online user security and encryption technology to safeguard your session with My BCH Portal.

    Can I view another person's health information?

    Yes. You can be granted access to another patient's medical records if that individual provides the hospital with completed, signed permission forms and you are 18 years of age or older.

    For access to a child's record, the parent or legal guardian can visit the Bacon County Hospital Medical Records department to obtain and complete a patient proxy access authorization form. The form can also be downloaded here. Authorization for proxy access to a child's record will automatically expire at the age of 18, and require the adult child to re-issue authorization with their own signature.

    Can I view health information related to care given through my doctor's office?

    My BCH Portal consists of health information specific to care received at Bacon County Hospital and does not reflect your doctor's office records. Bacon County Community Care Center and South Georgia Physicians Group offer access to online medical records here(Follow My Health).

    Please check with your doctor's office to determine their online health information services and policies.

    Who should I contact for additional questions?

    Call the medical records department at 912-632-8961 Ext. 1087 for any questions related to your portal access or authorizations.

    For any questions related to your medical record/clinical content, contact your physician.

    If you have an urgent medical issue, please call your physician's office, or in a medical emergency, call 9-1-1 or go to the nearest emergency department.
